Physical Therapy Evaluation

Includes a complete body diagnostic and assessment to understand the factors and mechanics behind your reason for seeking treatment.

Components of a full evaluation may include:

Physical Therapy Treatment Visits

Musculoskeletal, neurological, and pelvic health conditions are treated using a variety of tools including therapeutic exercise, therapeutic activity, neuromuscular reeducation, manual therapy, myofascial release, craniosacral therapy, electrical stimulation, cupping, and kinesiotaping.

Wellness Sessions

These sessions are tailored to your individual needs and goals with the focus of maintaining, improving, or aiding general health and well being. Common wellness sessions often include:

Strengthening and functional conditioning with long term conditions, like MS or stroke

Maintaining balance and agility in advanced age

Manual treatments including soft tissue massage, myofascial release, craniosacral therapy, and/or manual stretching.

Price: $150/ 55 minute session

Frequently Asked Questions

Do you accept insurance?

Yes, I am currently contracting with a company called Luna Physical Therapy to coordinate insurance billing for physical therapy evaluations and treatments. They accept most insurances, including Medicaid and Medicare. 

With that being said, there are restrictions associated with insurance coverage, including shorter sessions and one body part focus at a time. Treatments are solely physical therapy based, so the scope is slightly limited. 

Wellness sessions are not covered by insurance due to the larger scope and inclusion of alternative therapies. These visits are less restricted and focus on complete healing based on ALL of your needs. If you have a wellness benefit, I am happy to provide a superbill that you can submit for reimbursement.  

What forms of payment do you accept?

I accept payment through cash, check, and credit card. I am also affiliated with Venmo and PayPal. 

I am able to receive payments through HSA and CareCredit cards. 

Do I need a prescription from my doctor?

No. Washington is a direct access state for physical therapy services

What is your cancellation and rescheduling policy?

To cancel or reschedule your appointment, please notify me at least 24 hours in advance.

You can cancel by emailing ericabrook@embody.physio or calling/texting (425)282-8851. 

If you fail to give a 24-hour notice or miss your appointment, you may be assessed a cancellation/no show fee equal to the value of the visit booked. The cancellation fee will be charged to your credit card on file or charged prior to beginning your next session.

Do you have a physical location?

No, I operate primarily through home-based visits and virtual calls. 

Physical therapy and wellness clients receive concierge visits in their home (if in Snohomish or King County).

Coaching visits are conducted virtually, via Zoom.
